My object is to get some input on how I am coming across to my clients and also former clients who no longer send work my way. If you could take a peek and give me input on other questions to ask, I would appreciate it.

If you could take a moment to participate in our survey we would appreciate it. The purpose is to get honest, anonymous criticism and feedback so we can see where we need to improve and get your thoughts on how we can serve you better. We promise to keep this short and sweet.

Rate us from 1-5 on our in the following areas:
1. Creative Product (1=needs improvement 5=superb)
2. Branding and Marketing Strategy (1=needs improvement 5=superb)
3. Attention to Detail (1=needs improvement 5=superb)
4. Listening and Reacting to Input (1=needs improvement 5=superb)
5. Project Turnaround and Delivery (1=needs improvement 5=superb)
6. Prepress/Print Production (1=needs improvement 5=superb)
7. Estimating Process (1=needs improvement 5=superb)
8. Billing Process (1=needs improvement 5=superb)
9. Project Pricing (1=too expensive 5=very reasonable)
10. Print Pricing (1=too expensive 5=very reasonable)

One more question -- this is a classic I use in every interview I do. "Is there anything I should have asked you, but didn't?" This gives the survey recipient the opportunity to say anything they want. It is a little more compelling than "Comments" and often makes clients/subjects really think about what they want you to know.

Actually we wrote an online survey for the Greater Dallas Restaurant Association and wrote a backend so we could create our own surveys for other clients and ourselves. The GDRA has been dragging their feet so I am trying to get some use out of it.

I think it's fine--you want to keep it short and sweet, so people actuall reply, right? How are you doing this survey? Internet or print?
